Position Description

Cambridge Health Alliance (CHA) is seeking a highly skilled and dedicated Licensed Practical Nurse (LPN) passionate about making a difference in the lives of senior adults to join our dynamic interdisciplinary team at CHA PACE.

The PACE model, or Program of All-Inclusive Care for the Elderly, is considered the gold standard for community-based integrated care, as it allows seniors with chronic care needs to maintain mobility, independence, and quality of life in their homes and communities for as long as possible. Our high-functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services into the care plans of our participants.

In this rewarding role, you will provide direct nursing care to elderly participants both in their homes and in our PACE day clinic, ensuring continuity and quality of care across all settings.

Key Responsibilities:

  • Deliver hands-on nursing care in participants’ homes and at PACE clinic settings, in collaboration with the RN and PACE interdisciplinary team (IDT)
  • Assist in the assessment, planning, and implementation of individualized care plans
  • Utilize evidence-based practices to support participants’ physical and emotional well-being
  • Identify and communicate significant changes in participants’ conditions to the RN and IDT
  • Support clinic triage and provide cross-coverage as needed to ensure uninterrupted care delivery
  • Maintain accurate and timely documentation of care and services provided

We are currently offering a $5,000 sign-on bonus for newly hired PACE LPNs!

Qualifications:

  • Graduation from an accredited Licensed Practical Nursing program
  • 1-year experience in geriatrics preferred
  • 1- 2 years home care experience preferred
  • Maintains current Basic Life Support (BLS) certification
  • Knowledge of a second language preferred
  • Must possess a valid US driver's license and access to a car for home visits
