Job Details

Registered Nurse Case Manager:

The Clinical Case Manager will work with an interdisciplinary team ensuring continuity of care, managing and modifying treatment plans, discharge planning, reviewing and analyzing case management reports, etc.

* Active MA license
* Prefer a Reistered Nurse with 2 or more years related clinical experience
* Experience with discharge planning
* Certified Case Manager (CCM) preferred
* BLS Certified
